As first reported by our sister site MMW, Qualcomm Incorporated has just announced that its subsidiary, Qualcomm Life, Inc., is launching the HealthyCircles Mobile application.

According to details provided via email, the application is a secure, medical-grade mobile care solution that provides a frictionless on-the-go care experience for consumers via their mobile device.

The application is an extension of Qualcomm Life’s web-based HealthyCircles Care Coordination Platform, offering personalized care, patient activation and behavior modification for chronic care patients.

“HealthyCircles Mobile is an adaptive, personalized mobile care experience for patients who need it the most: people with chronic conditions and those transitioning from hospital to the home,” said James Mault, MD F.A.C.S., vice president and chief medical officer, Qualcomm Life, Inc. “Our goal is to provide a frictionless platform that will guide healthy behaviors and motivate patients to engage in their own self-care.”
